El hombro congelado, también llamado capsulitis adhesiva, causa dolor y rigidez en el hombro. Con el tiempo, el hombro se vuelve muy difícil de mover.
El hombro congelado se produce en aproximadamente el 2% de la población general. Esta lesión afecta a personas entre las edades de 40 y 60 años, y se produce en las mujeres con más frecuencia que los hombres.
*ANATOMÍA
El hombro es una articulación formada por tres huesos: el hueso del brazo (húmero), el omóplato (escápula) y la clavícula.
La cabeza del húmero encaja en una cavidad poco profunda en el omóplato. La articulación la rodea un tejido conectivo fuerte, llamado la cápsula del hombro.
Para facilitar el movimiento del hombro, la capsula del hombro y la articulación son lubricadas por el líquido sinovial
*DESCRIPCIÓN
En el hombro congelado, la cápsula del hombro se engrosa y se vuelve dura. Se desarrollan bandas rígidas de tejido – llamadas adherencias -. En muchos casos, hay menos líquido sinovial en la articulación.
El signo distintivo de esta condición es no poder mover el hombro – ya sea por su cuenta o con la ayuda de otra persona. Se desarrolla en tres etapas:
1ª etapa: Congelación
En la fase de «congelación», poco a poco se va teniendo más y más dolor. A medida que el dolor empeora, su hombro pierde el rango de movimiento. La etapa de Congelación suele durar de 6 semanas a 9 meses.
2ª Etapa: Congelado
Los síntomas dolorosos pueden realmente mejorar durante esta etapa, pero la rigidez permanece. Durante los 4 a 6 meses de la etapa de «congelado», las actividades diarias pueden ser muy difíciles ya que el rango de movimiento cada vez se ve más limitado.
3ª etapa: La descongelación
El movimiento del hombro mejora lentamente durante la fase de «deshielo». Conseguir volver a la normalidad o acercarse a la resistencia y al movimiento normal suele durar de 6 meses a 2 años.
En el hombro congelado, los suaves tejidos de la cápsula del hombro se vuelven gruesos, rígidos, y se inflaman.
*CAUSAS
Las causas del hombro congelado no se entienden completamente. No existe una clara conexión con el dominio del brazo o la ocupación. Algunos factores pueden poner más en riesgo de desarrollar este problema:
La diabetes. El hombro congelado se produce con mucha más frecuencia en las personas con diabetes, que afecta a 10 % y el 20 % de estos pacientes. La razón de esto no se conoce.
Otras enfermedades. Algunos problemas médicos asociados con el hombro congelado: hipotiroidismo, hipertiroidismo, enfermedad de Parkinson y enfermedades cardiacas.
La inmovilización. El hombro congelado se puede desarrollar después de que un hombro ha sido inmovilizado por un período de tiempo debido a la cirugía, una fractura u otra lesión. Hacer que los pacientes muevan sus hombros poco después de una lesión o cirugía es una de las medidas prescritas para prevenir el hombro congelado.
El objetivo del tratamiento es controlar el dolor y restaurar el movimiento y la fuerza a través de la fisioterapia.
Ejercicios específicos ayudarán a restaurar el movimiento. Lo ideal es que estos ejercicios estén bajo la supervisión de un FISIOTERAPEUTA. La terapia incluye estiramientos o ejercicios para ampliar el rango de movimiento del hombro. A veces se utiliza calor o aparatos de electroterapia para ayudar a relajar el hombro antes de los ejercicios de estiramiento.
En algunos casos, es necesaria la cirugía. No obstante, tras la cirugía, la rehabilitación es imprescindible para mantener el movimiento que se logró con la cirugía.
Los tiempos de recuperación varían, de 6 semanas a tres meses. Aunque es un proceso lento, el compromiso con la terapia es el factor más importante para volver a realizar todas las actividades que por el hombro congelado dejamos de realizar.
Aunque es poco frecuente, el hombro congelado puede ser recurrente, especialmente si un factor que contribuye como la diabetes sigue estando presente.
